SLIDING SCALE COMMUNITY CARE

Services at Mid-State are available on sliding scale to all patients who qualify. Please feel free to contact our Community Care Coordinator for more information.

The Community Care Coordinator provides assistance for patients who are uninsured or underinsured and wish to apply for sliding fee scale at Mid-State. Our Community Care Coordinator is available to provide assistance with a Community Care Application, a Health Care Marketplace Application, prescription assistance, payment plans and other programs to assist you. The goal is to help make your health care costs here at Mid-State affordable.
